Health Forms
Families are required to have an updated health form for the 2024-25 school year. Forms should be returned to your school's office.
Waiver of Immunization Requirement: Parents seeking to exempt their children from Minnesota's vaccination requirements are advised to connect with your school's main office. The school's health office will provide guidance about obtaining, completing, and notarizing the form for Immunization Exemption.
If your scholar has any health needs please complete the appropriate forms, these forms can be faxed from your clinic or brought to school.
